CCQ delivers the primary health network (PHN) program for Central Queensland, Wide Bay and Sunshine Coast on behalf of the Commonwealth Department of Health. CCQ has an ambitious agenda to design and deliver better primary health outcomes for the people in their region. Achieving this will require CCQ as a commissioning organisation to not only focus on primary health service provision but also on the environmental, social and cultural determinants of health which support communities to live their best lives.
CCQ’s diverse region covers 161,108 km2 from Sunshine Coast to Rockhampton and west to Emerald. The population in the region is just under 1M people with around 68% living regionally or remotely and more than 28% living in the most disadvantaged quintile. CCQ collaborates with the community to provide exceptional primary healthcare services to reduce health inequalities for the region. CCQ has seven key priorities for health improvement: mental health, alcohol and other drugs, Aboriginal and Torres Strait Islander health, population health, health workforce, digital health and older person’s health.
As Healthy Ageing Coordinator you deliver CCQ’s healthy ageing programs, projects and initiatives at a local level. This role is based in the Wide Bay area and the successful person will need to be based in this region.
You will partner with local stakeholders to enhance healthy ageing and person-centred health and ageing services and facilitate quality improvement in aged care and primary health care services guided by community needs as well as local, regional and national strategies.
Key to your success will be the building of productive relationships with stakeholders and facilitates integrated care across acute, residential, primary health care and community settings to influence older persons’ services to adopt person-centred interdisciplinary models of care and digital health initiatives.
To be considered for this opportunity you will need to be a registered health practitioner with a strong understanding of primary health and aged care with demonstrated experience building collaborative stakeholder relationships. You will be dynamic and outcomes focused with the proven ability to manage multiple projects and a range of stakeholders.
What is offered:
- An attractive remuneration package, including salary sacrifice packaging, up to $15,900 per year tax free.
- $95,000.00 to $109,000 p.a. salary range based on experience + Super
- You can also use up to $2,650 of your pre-tax income to pay for entertainment expenses such as meals and holidays.
